There are two techniques for hair transplant.

FUT hair transplant: We cut the skin with a knife in this method. We take skin from the back of your head. The length of this skin is 25 – 30 centimetres (from left to right ear), and the width is approximately 1.5 to 2 cm. The wound that forms on the back side of your head due to the removal of skin is stitched with the help of 40 to 50 stitches. After that, we separate hair grafts from that skin strip. Then the surgeon makes holes (slits ) in your bald area. Then assistant inserts the hair grafts into the holes created by the surgeon. The FUT method requires cuts and stitches, so this method is not popular nowadays with patients.

FUE hair transplant – This method is called follicular unit extraction. As the name suggests, in this method, we remove follicular units (hair grafts) from your donor area with the help of a sharp punch. A tiny wound is formed, which heals in 2 to 5 days. Although it is a desirable method by all patients but this technique requires a highly expert surgeon. The FUE process may do a lot of damage to the donor area if not done correctly. After extraction of grafts, the surgeon makes holes (slits) in the bald area, and assistants insert the grafts in these slits.

As indicated above, there is only one difference between FUE and FUT hair transplants. That is the extraction process of hair grafts. Slit-making and graft insertion is common in both techniques.

In a FUT technique, the time taken for the strip removal is around 1 hour, But the extraction process in an FUE hair transplant takes 2 to 5 hours, depending on the number of grafts. As FUE graft extraction takes a lot of time, multiple hair surgeries are not possible by the surgeon when he performs FUE hair transplant. FUT provides an opportunity to perform multiple hair transplants in a day so many surgeons do their best to promote FUT technique. 

Hole-making ( slit pattern making) takes around 1 hour in both techniques. In most good clinics, slit-making is done by a surgeon.

Assistants usually insert grafts into the slits after that, so the surgeon’s role ends after making slits in your bald area. If you want a surgeon to insert grafts also then the process becomes lengthy and costlier for you. You can request that step also to the surgeon and get his quotation.

You will be surprised to know that most of the commercially successful clinics are getting your graft extraction done by their assistants who are nursing degree holders. They are not doctors by qualification. Surgeons operate VIPs and celebrities. Surgeons advertise their clinic and get your surgery done by their assistants.

2. Surgeon plans and does holes (slit-making) in the bald area to decide hair direction and density. The rest work is done by assistants. Surgeons time consumed is 1 – 1.5 hours in a patient. He can operate 3 – 5 hair transplant surgeries in a day with this methodology.
3. Surgeon plans , makes slits and do graft extraction also himself. The assistant job is to place grafts in slits – This consumes 3 – 5 hours of a surgeon in one hair transplant surgery. He can perform one big hair transplant case or two small cases by this methodology.
4. Surgeon plans , makes slits , extract grafts and insert grafts also in recipient area . One surgery takes 6 – 10 hours of surgeon . he can do small case in one day or one major case in two days .

So you now know that reducing the surgeon time increases the number of cases performed by hair clinic. this means more income . Thats why most clinics want to reduce surgeons time in your hair transplant.

Why A surgeon must do follicular extraction

Before and after the hair transplant result does not show you the damage done to the donor area by the assistant. Assistants can damage an additional 30 % to 80% grafts from your donor area to extract same amount of grafts. It may not impact the hair transplant result, but the damage is permanent. Whenever you need more grafts for any future hair transplant, this damage will prevent the future surgeon from taking a good number of grafts from your donor area. But clinics can hide this fact easily as most patients compare only before and after hair transplant photos or videos of the patients. 

Clinics establish their authority using celebrity photographs, hair society memberships, good social media activity, and dense results. But all this is insignificant if the surgeon does not do follicular unit extraction. You often get to know about this only on the day of surgery. At the time of booking, the clinic’s marketing executive remains silent regarding the surgeon’s role in your surgery or gives you false statements that the surgeon will do your hair extraction himself.

You have to make sure that the clinic has the policy that the surgeon always does graft extraction because that is the only difference between an FUE and FUT SURGERY.
