WHAT IS DIRECT HAIR TRANSPLANT? – HOW IS IT DIFFERENT FROM NORMAL FUE?​

Direct Hair Transplant is a modification of the FUE Technique, which aims to decrease the time of graft out of the body. Read the FUE Technique first to understand this technique.

Why does the survival rate of hair grafts decrease when we keep grafts outside the body

When hair grafts are removed from your donor area, they lose their blood supply and oxygen supply. When grafts are inserted in the bald area, they get oxygen and nutrients by plasma imbibition. During this process, the donor tissues receive their nutrition through the absorption of plasma from the recipient wound bed via capillary action. Blood supply creation takes a few days.
But placing the grafts into slits of bald area takes time. During this time, they are stored in normal saline solution or specific solutions. The longer the grafts are stored outside the body, the more injury occurs to them due to depletion of the oxygen supply .

LIMMER STUDY

Limmer conducted a study, which is quoted by all hair transplant research papers, for the graft out-of-body time impact on graft survival rate. It is quoted below in the diagram

As you can see that, 4 – 6 hours out of body time decreases the survival rate below 90 %. Further studies were done and concluded that after 2 hours of storage, the survival rate may be affected.

Few other studies also concluded that graft survival rate after 2 hour of storage can be effected up to 30 % also in some patients.

Our Experience

Practically, we have not seen any patient in normal FUE to loss 30 % grafts even after 4 – 5 hours of storage outside the body, so studies indicating that it is a major factor to get survival rate may be biased. But it can impact 10 – 15 % results in longer surgeries.

What we do in DHT process

In Normal FUE, the Hair is harvested from the donor area and Kept in normal saline. Suppose 3,000 grafts are planned for you. First, these 3000 grafts will be harvested from your donor area and kept stored in normal saline. After that, the implantation of grafts is started in the bald areas. This may require about 3  – 6  hours of storage in saline / Plasma.  So every hair graft remains outside the body for 3 – 6 hours.

In Direct Hair Transplant, we do the slitting pattern (first holes making is done), first in the bald area. The hairs are harvested in the same technique as FUE, but every graft is implanted immediately or within 10 – 60 minutes in the bald areas. So, Grafts either don’t stay outside the body for a prolonged period, like 2 – 6 hours. It reduces ischemic injury to grafts, leading to better graft survival rates.
