It is foolish to assume the expert. Hair transplant results come at the expense of your donor area; they damage an additional 30 to 50 percent of the donor area to get the same result. Suppose you hire them to extract 3,000 grafts; they will damage around 900 to 1,500 extra grafts to extract those 3,000 grafts.

So, it is not true that donor-area follicle excision is an easy process. Even expert doctors only become proficient in donor-area extraction after three or four years of independent practice.

After we started seeing the donor area through a microscope instead of a surgical loop, we were able to handle even the most difficult hairs, like curly hairs, more confidently than ever before, especially if the patient had thin or curly hair. This technique was very useful, and even in normal patients, it improved the graft selection, identifying which grafts were thicker and stronger.
Because of this, we can extract with smaller punches and can accurately extract even in complex cases. Partial transactions are also reduced, so you get stronger, thicker hair. Dr Akhilesh has completed more than 1,000 hair transplant cases with the help of a surgical microscope at 10x to 16x magnification.
He is the only doctor in India who routinely uses a microscope for donor-area hair follicle extraction. Except for him, we don’t know of any other practical doctor who routinely uses a surgical microscope in FUE practice in India. However, when patients ask other surgeons if there is any benefit to this, other surgeons fail to recognize its significance because they need to do it themselves. But the fact is that most surgeons do not remove the grafts from the donor area themselves, and here the doctor is, so he has a much better idea of how difficult graft extraction is, and this exercise is to ensure better quality grafts and less donor area damage during the graft extraction.
The third thing Dr. Akhilesh worked on is a multiphasic graft extraction motor, and he has made his own multiphasic graft extraction motor, which uses rotation and oscillation movements. Although machines available in the market like the Triveni, Wow machine, and others also work on the same principle, they are timer-based. In this MSDF machine, he changes the rotation and oscillations with the help of a foot switch after seeing the depth with magnification. He decides for each graft how much depth should be there before changing the rotation to oscillation. This gives more control for the follicle unit extraction process when we use multiphasic graft extraction, and we can go deeper than usual to extract the grafts by this technique.
